  2. Dates of medical treatment (including doctor visits, hospitalizations, lab work, and medical tests);
  3. Personal information, including dates of prior marriages; AND
  4. A detailed work history for the past 15 years, including the names, addresses, phone numbers, and dates of employment for each job.
